Tuna Helper is an extension that is unknown to many, mainly due to its status as an invisible extension. It is present only if the System 7 Tune-Up 1.1.1 was installed on a Macintosh running System 7.0 or 7.0.1.

Under these earliest versions of System 7, an error would occur where folders and documents began disappearing after repeated operations in special procedures dealing with floppy disks, folders, and the desktop. The Tune-Up corrected this issue (but did not restore any lost icons).

Tuna Helper is located within the System file; it is not in the Extensions folder. It cannot be removed after Tune-Up 1.1.1 is installed on a Mac.[1]
